About Our Client:
Our client is an ambulatory centre in Richmond Hill.
Position Scope:
The Systems Applications Pharmacist contributes to overall patient care through involvement in guiding the selection, design, implementation, and evaluation of medication automation systems and pharmacy applications, conducting staff training and support on systems, as well as participating in the provision of patient care, medication distribution, drug information and educational programs in the inpatient setting, as assigned. You are also a great relationship builder, communicate effectively with peers across the entire organization and effortlessly connect with physicians, staff, patients, and families. You are highly organized, have great interpersonal skills, demonstrate critical thinking and reasoning skills and adapt well in a dynamic environment. All roles are essential in providing a safe and healthy environment for patients, staff, physicians, volunteers and others by ensuring that health and safety policies are followed.
Responsibilities:
- Provides all activities related to the selection, system design, implementation plan, implementation, testing, and evaluation of all medication automation and pharmacy systems and applications.
- Works close with Pharmacy Staff and Clinical Informatics regarding the integration of all medication automation systems and applications, ongoing maintenance and system upgrades, as well as the design and implementation of web based applications pertaining to Pharmacy Services.
- Conducts training and provides support for staff on systems, devices, application and equipment.
- Participates in re/designing pharmacy processes and workflow to improve quality and efficiency.
- On assigned basis, provides patient-oriented clinical services – medication order review, medication profiling, pharmacotherapy monitoring, interdisciplinary participation, drug use evaluation, adverse drug reaction reporting, medication counseling, medication incident reporting and follow-up, and medication history service.
- Maintains activities in all drug use control functions-drug distribution systems, computerized medication order processing system, formulary system, inventory control system and drug procurement.
- Participates in drug information and other educational activities within the department, on the nursing units and throughout the centre.
- Responsible for the activities of the pharmacy technician assigned on a day to day basis.
- Follows all Federal and Provincial rules and regulations set to control the dispensing to the general public and the distribution of medications, and to provide high quality services within the Centre.
- Responsible for working safely in accordance with the Occupational Health and Safety Act.
Required Skills and Knowledge:
- Demonstrated knowledge and skills in pharmaceutical care, disease management and clinical pharmacology.
- Prior experience with pharmacy/medication automation software and/or implementations would be an asset.
- Demonstrated written and verbal communication skills.
- Demonstrated pharmaceutical skills and techniques.
- Demonstrated skills in computerized pharmacy system.
- Demonstrated people skills in client/customer service.
- Knowledge of pharmaceutical care process, disease management and clinical pharmacology
- Familiarity with evidence-informed practice
- Demonstrated organizational skills.
- Demonstrated critical thinking and deductive reasoning skills.
- Adaptability and flexibility in a dynamic environment
- Deductive reasoning, attention to detail and medication safety.
- Bilingual French/English an asset.
Education and Experience:
- Graduate from a recognized university Pharmacy Program (B.Sc. Pharmacy).
- Licensed under Part A of the Ontario College of Pharmacists.
- Previous clinical hospital experience or hospital residency is an asset.
- Experience with Meditech an asset.
